Proof of residency requirements

District 68 is dedicated to the academic success of all students within our attendance boundaries. To meet our district requirements, proof of residency is required from the listed categories below. This paperwork will be kept on file in the business office. This process is also repeated if there is a change of residency status at anytime during the school year. Letters will be mailed home to each family who falls into the following categories.

Parents of students needing to provide Proof of Residency include:

• Students new to the District

• Students starting kindergarten in the fall of 2012

• Students currrently enrolled in District 68 who will be: Entering fifth grade in the fall of 2012; renting/leasing their home; living with a family in District 68 under an approved Affidavit Wavier.

Information will be included in your registration packet.

A letter of information will be mailed home in the late spring.

Please come in person to submit the correct documentation in your category: June 4 - junior high library, 9 a.m.-3 p.m., fifth grade, kindergarten and new families; Aug. 6 - junior high office, 8 a.m.-2:30 p.m., renting and affidavits.

Please do not mail your documents to the school or send them in with your students. These documents have private information on them and could get lost. Questions can be directed to Mrs. Azzato, (847) 367-4120, ext. 2038.
